Before your appointment, you will need to do the following:
- Complete an account application and fax a copy of your work ID badge or current check stub. Applications are also available in our office or may be emailed or faxed to you per your request.
- Retired employees:
- Employees that have retired from a state agency can provide a copy of their Blue Cross Blue Shield medical card since it states the person is retired. An account application also needs to be completed.
- Employees that have retired from local government need to contact the Retirement Services Division of the NC Department of State Treasurer at 877-627-3287. Have them fax a Verification of Income letter to Customer Service at 919-324-6238. Or you may log into your account at www.myncretirement.com for that request.
- Have a current, valid eyeglass prescription from your eye doctor.
At your in-person appointment, you will be properly fitted for your glasses and payment must be made in full at that time. We accept payment via personal check, debit or credit card (Visa/Master Card), or a certified money order. You may also use your Flex Spending Card if you are involved in this program. Most eyeglasses take three weeks to be manufactured. We accept the following vision insurance: EyeMed through NC Flex and Community Eye Care.
